My biggest fear isn't losing money, but at the end of the year, flipping through transaction records until my eyes go straight and they don't match... So now I try to "keep track while walking." Not aiming to be overly professional, at least: every deposit and withdrawal, on-chain transfer, coin swap, participation in activities, take a screenshot, and casually toss the transaction hash into a spreadsheet, with notes in plain language (what it's for, from where to where). The exchange will periodically export a history record, don't expect to be able to pull out a complete one later.
